Le GCS TéléSanté Centre a été créé en avril 2009 par les principaux établissements de santé publics et privés de la région ainsi que l'Union Régionale des Médecins Libéraux. Depuis notre création nous augmentons chaque année le nombre de nos adhérents et sommes maintenant au service d'établissements médico-sociaux et de l'Union Régionale des Professionnels de Santé (pharmaciens, kinésithérapeutes, chirurgiens-dentistes, infirmiers).

Nous avons pour but de proposer un panel de services mutualisés permettant à nos membres d'améliorer leurs systèmes d'information et ainsi, favoriser la coordination des soins et la bonne prise en charge du patient. C'est pour servir ce but que nous sommes maîtrise d'ouvrage c'est-à-dire que nous pilotons les actions et projets de systèmes d'information à la demande de nos adhérents mais aussi de l'Agence Régionale de Santé (ARS) ou dans le cadre d'appels à projet lancés par le Ministère de la Santé.

La Plateforme Santé Douaisis est un lieu d'accueil, de conseil et d'accompagnement pour les patients (et leur entourage) concernés par :

- Une ou plusieurs maladie(s) chronique(s) (obésité/surpoids, diabète de type 2),

- Un cancer,

- Une fin de vie à domicile.

4 champs d'action :

- La Santé Publique: évènements grand public sur la prévention, repérage et au dépistage communautaire itinérant des problématiques de santé auprès des populations vulnérables.

- L'offre de soins: coordination des parcours de santé en appui des professionnels de santé, programmes d'Education Thérapeutique du Patient, évaluation médico-psycho-sociale du patient et élabore avec le médecin traitant un Plan Personnalisé de Santé (PPS) et planification et le suivi des actions proposées.

- L'offre sociale et médico-sociale: accompagnatrice des allocataires du RSA, assistante sociale afin d'accompagner les patients dans les situations dites complexes. Chaque fois que cela est possible, l'équipe veille à orienter le patient vers d'autres services ou structures relais afin de favoriser l'autonomie du patient.

- L'amélioration des pratiques professionnelles La Plateforme Santé Douaisis a mis en place un programme de formation intitulé : « FETE, se Former Ensemble pour Travailler Ensemble.

O Centro Universitário São Camilo ministra cursos nas áreas da saúde, gestão e educação. Graduação, Pós-Graduação, MBA, Mestrado, Doutorado e Pós-Doutorado.
